New Patient Registrationregister_now

To apply to become registered with our practice, you should call at the desk and ask for an application form or bring in your current medical card. Forms can also be downloaded from the above "Registering Online" tab.

When you register with Grange Family practice you will be registering with the practice, not an individual GP. You can however request to see a particular GP at any time or different GP's for a particular case.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
